Typusfundort: Virgin Islands (USA), St. Croix, vor der Nordküste des Buck Island Reef National Monument, 17.78925°N, 64.61789°W (Karte), 1–3 m Tiefe.
Etymologie
Das spezifische Epitheton quadra (lateinisch für "quadratisch") basiert auf der charakteristischen quadratischen Form des dunklen Balkens auf dem Schwanzstiel.
Verbreitung
Typus-Exemplare und zusätzliches Material wurden von den Bahamas, den Jungferninseln (USA) und Antigua sowie in der Westkaribik gesammelt: in Yucatán, Mexiko (als Larve), Honduras und Belize sowie in den Offshore-Inseln von Providencia. Es scheint, dass die Art noch nie unter Wasser fotografiert worden war.
